Family Connection, a program of Family Services, Inc., is designed to assist adults and their families with long distance caregiving. Family Connection will assist families in determining, arranging, and providing needed care for their loved ones. Although primarily geared toward the elderly, this service can provide assessments, recommendations, and care arrangements for any individual experiencing an illness or disability.

The components of Family Connection are:

Multi-dimensional Assessment and Care Plan: An initial assessment is provided by a Geriatric Care Manager in consultation with a Nurse. This assessment will include home visits as well as consultation with physicians and other service providers. The Geriatric Care Manager will make recommendations to the adult and their family and is available to assist with setting up needed services.

Care Monitoring: Monthly care monitoring of the individual in their home or in the supportive living facility where they reside can be arranged.

Selecting a Nursing or Supportive Living Facility: For short term recuperation or long term care, the Geriatric Care Manager will seek out and investigate facilities most appropriate to the specific needs o fthe individual and family.

Moving Coordination Service: This service includes home preparation for sale, referral to a real estate agency, cleaning and packing, unpacking and placement of furnishings and belongings, and all other services necessary to support the individual in settling comfortably into their new residence.

Escort to Appointments: Family Connection staff are available to escort individuals to medical appointments or other errands outside the home.

Personalized Services: Other services are available as needed by the individual. Some examples are assistance in keeping financial or insurance records or assistance with bill paying.

It is possible to give your family member that needed attention, even though you can't actually be there. An important part of our service is the regular feedback we provide to family members regarding their loved one's well being. A monthly "Family Feedback Letter" is provided to give families reassurance and information about the individual's condition and needs.
